Nurse Clinician, Infectious Disease Clinic and HIV/Program - (Paid Relocation to New Hampshire)
The InfectiousDisease Clinic at Dartmouth Hitchcock Medical Center is seeking a highlyqualified HIV Nurse Clinician to work with our patients to navigate resourcesand manage their disease with PrEP and PEP anti-viral treatments. This rolewill require 60% phone triage with patients, 10% work with committees onprograms and education and approximately 30% patient facing services in theclinics. Qualified candidates will have 5 years of experience with HIV Nursing,Substance Abuse and or Public Health and preferably have a MSN. We are seekinga well-rounded clinically strong RN who can work independently with a patientcase load. The RN in this role with have empathy, compassion, open minded anddemonstrate a commitment to working with marginalized and vulnerablepopulations of people with different cultural and sexual orientations.
Purpose:
Coordinates the interdisciplinary care of their patients. Ensures appropriate patient care and family involvement and education and school/community consultation.
Responsibilities: A listing of the key responsibilities
- Supports patient and families through procedures, medication therapy, emotional and financial issues, etc.
- Provides telephone triage, prescription needs, and notification and implication of lab results.
- Arranges for lab work and procedures. Provides timely notification to families of the results of test procedures, radiology studies, blood work, and any other tests ordered by the team.
- Assists with patient and family needs and patient education materials.
- Documents patient problems and progress.
- Participates in interviewing and evaluation of staff.
- Works with Nurse Manager to ensure adequate nursing coverage.
- Acts as multi-disciplinary liaison with staff from radiology, laboratory, financial services, pharmacies, medical insurance companies, referring providers, and the in-patient units.
- Assists with training and education of nursing students, medical students, and medical residents.
- Participates in relevant educational offerings to enhance effectiveness and achieve required in-service training, and certification.
- Performs other duties as requested or assigned.
Graduate from an accredited Nursing Program required.
Bachelor of Science Degree in Nursing (BSN) with 3 years of clinical experience required.
Masters of Science Degree in Nursing (MSN) strongly preferred.
Required Licensure/Certification Skills:
Licensed Registered Nurse in New Hampshire required.
BasicLife Support (BLS) Certificate required upon hire and as applicable, higherlevel per unit certification requirements as stated on the grid attached to theAHA and Life Support Policy.
Advanced Cardiac Life Support (ACLS) required for the OR-Anesthesiology Department
