Oncology Nurse Navigator RN - (Paid Relocation to New Hampshire)
NURSE NAVIGATOR ONCOLOGY
Nurse Navigators are play a critical role is establishing community services and locating resources within DH and outside DH to meet the needs of our cancer patients. Qualified candidates will have a BSN and (Masters preferred). This role works collaboratively with an interdisciplinary team of RN's, Providers, Researchers and clinicians within the cancer center. Three years of inpatient or clinic oncology experience is required and you will sit for your Nurse Navigation Certification within a year of accepting the role.
Purpose:
In collaboration with an interdisciplinary team,provides care coordination, education and support to patients, families andcaregivers throughout the cancer continuum to ensure high quality care across all caredelivery sites.
Key Responsibilities:
1. Assessespatient needs upon initial encounter and periodically throughout navigation,matching unmet needs with appropriate services and referrals and supportservices, such as dietitians, providers, social work, and financial services.
2. Identifiespotential and realized barriers to care (e.g., transportation, childcare, eldercare, housing, language, culture, literacy, role disparity, psychosocial,employment, financial, insurance) and facilitates referrals as appropriate tomitigate barriers.
3. Developsor utilizes appropriate assessment tools to promote a consistent, holistic planof care.
4. 4. Facilitatestimely scheduling of appointments, diagnostic testing, and procedures toexpedite the plan of care and to promote continuity of care.
5. Provideseducation to patient, families, and caregivers to the cancer healthcare system,multidisciplinary team member roles and available resources.
6. Participatesin the tracing of metrics and patient outcomes, in collaboration withadministration, to document and evaluate outcomes of the navigation program andreport findings to the cancer committee.
7. Assistsin the identification of candidates for molecular and/or genetic testing andcounseling and facilitates appropriate referrals.
8. Actsas a liaison between the patients, families and caregivers and the providers tooptimize patient outcomes.
Performs other duties as required or assigned.
Minimum Qualifications:
ü Graduatefrom an accredited Nursing Program required.
ü Bachelor of Science Degree in Nursing(BSN) required; Master's preferred.
ü Three years of oncology experience in anacute or ambulatory care setting preferred.
ü Oncology Nurse Certification (OCN)required within one year of hire.
ü Nurse Navigation Certification requiredwithin two years of hire.
ü Strong organizational skills and theability to prioritize and reprioritize
ü Ability to develop collaborativerelationships internally and externally
ü Strong oncology knowledge
ü Basic computer skills
ü Ability to work autonomously
Required Licensure/Certification Skills:
ü Licensed Registered Nurse in NewHampshire required.
: Basic Life Support (BLS)Certificate required upon hire and as applicable, higher level per unitcertification requirements as stated on the grid attached to the AHA and LifeSupport Policy.
